Medicaid work requirements in 2027: what agents should know

Medicaid work (community engagement) requirements start by Jan 1, 2027 for adults 19 to 64 in the ACA Medicaid expansion group. People who do not show compliance are also barred from ACA marketplace premium tax credits, so they are not an easy ACA sale.
Who do the requirements apply to?
Adults at least 19 and under 65 in the expansion group (and similar section 1115 waiver groups), with exemptions such as pregnancy and Medicare enrollment (source; source).
When do they start?
By Jan 1, 2027; a state may start sooner. HHS can grant a state a good-faith exemption, which must end no later than Dec 31, 2028 (source).
Can people who lose Medicaid buy subsidized ACA coverage?
Not if they lost it for failing the requirement: CMS says such a person is precluded from advance and final premium tax credits (source). People who lose Medicaid for other reasons, such as higher income, may still qualify; check each case.
